CHAP is an organization approved by the Centers for Medicare and Medicaid Services (CMS) to establish standards for care quality, operational management, health outcomes, and more. CHAP also recommends program certification to Medicare.
Next Level Home Care: Remote Patient Monitoring
Munson Healthcare Home Care is the only organization in northern Michigan to offer remote patient monitoring telehealth services.
Remote monitoring is a safe, secure, and highly convenient alternative to doctor’s office and home visits. Using a tablet computer, patients are asked several questions every morning. Questions might include how they’re feeling, their heart rate, blood pressure, and weight. Responses go through a cellular network to an online portal monitored by a home care registered nurse.
When needed, the nurse may suggest an in-person visit to follow up on any concerns. Patients can also request a face-to-face appointment with the push of a button.
