For Your Patients

  • As part of your normal care conversations, please ask all adult patients (18+) if they have an advance directive.

  • Have someone available in your office to have ACP conversations with patients, such as a care manager.

  • Print or order pre-printed ACP documents for your patients during their visit.

    • To order pre-printed ACP materials (advance directive booklet, ACP workbook, MI-POST out of hospital medical order, etc.) for use in your office, submit a McKesson e-requisition or fax an order form.

  • Patients can attend a free, virtual ACP workshop to get them started. Click here for a list of workshops and to register.

  • Unable to attend an ACP workshop? Patients can watch a video on "Advance Care Planning — Understanding Durable Power of Attorney & Patient Advocate," which is available here.

For Healthcare Professionals 

  • Receive reimbursement for ACP conversations by using the time-based CPT Codes 99497 and 99498. 

  • Attend Virtual ACP Training for Healthcare Professionals (here's a printable flyer)
    • Description: This training provides an in-depth understanding of ACP process that enables healthcare professionals to conduct structured, meaningful goals of care conversations with patients.
    • Who: Any healthcare professional who will conduct ACP conversations with patients and families and/or lead ACP program development.
    • When: Wednesday, Sept. 27, 10 am – 12 pm | Register
    • CME: This training has been approved for a maximum of 2 AMA PRA Category 1 Credits™.
