Starting Tuesday, Aug. 15, Otsego Memorial Hospital (Gaylord) will offer outpatient PICC line insertion. 

To order, fax OMH Outpatient PICC Line Insertion referral form to 989-731-7732. Referral forms can be found here.

Please provide patients with PICC Line Patient Education.
