2022 Veteran's Day Recognition

On November 11, we honor our veterans and active military for their service and their sacrifice. If you are a veteran or have a family member who is a veteran or on active duty, and would like to be included in our internal and external recognition, complete the form below and attach a photo by Wednesday Nov. 2.

**Please note, if you submitted a photo last year, that photo will be used again in this year’s video. 

By submitting this information, you are authorizing Munson Healthcare to use your name, department, military branch and photograph in internal and external communications including social media.
