National Healthcare Decisions Day, celebrated annually on April 16, is an important reminder to encourage people to complete an advance care directive.

For Your Patients

  • As part of your normal care conversations, please ask all adult patients (18+) if they have an advance directive.
  • Have someone available in your office to have ACP conversations with patients, such as a care manager.
  • Have ACP documents readily available for patients during their visit. To order ACP materials, click here.
  • Patients can attend a free, virtual ACP workshop to get them started. Click here for a list of workshops and to register.

For Healthcare Professionals 

  • Receive reimbursement for ACP conversations by using the time-based CPT Codes 99497 and 99498. 
  • Attend Virtual ACP Training for Healthcare Professionals
    • Description: This training provides an in-depth understanding of ACP process enabling healthcare professionals to conduct structured, meaningful goals of care conversations with patients.
    • Who: Any healthcare professional who will conduct ACP conversations with patients and families and/or lead ACP program development.
