National Healthcare Decisions Day, celebrated annually on April 16, is an important reminder to encourage people to complete an advance care directive.
For Your Patients
- As part of your normal care conversations, please ask all adult patients (18+) if they have an advance directive.
- Have someone available in your office to have ACP conversations with patients, such as a care manager.
- Have ACP documents readily available for patients during their visit. To order ACP materials, click here.

- Description: This training provides an in-depth understanding of ACP process enabling healthcare professionals to conduct structured, meaningful goals of care conversations with patients.
