MMC Geriatric Consults

Munson Medical Center is now offering a hospital-based Geriatric Consultative Service to address the unique needs of geriatric patients with compassion and expertise. The team, led by Jonathan Beaulac, DO, FACOI, will work alongside patients’ providers to provide consultation.

Hours: Monday – Friday, 9 am – 5 pm

How to request a consult:

1. Place a consult order in Oracle PowerChart/FirstNet without a specialty.

2. Add Dr. Jonathan Beaulac to the consult.

3. Page Dr. Beaulac via PerfectServe Clinical Collaboration. 
   Note, a Geriatric Service order will be added to Oracle after the OPR Go-Live on Jan. 25. 

More information:

• The service is available for hospital-based consultation only. Telehealth access is planned for our community hospitals in the future.

• It is not a primary service, and patients will not be admitted to this service.

• Conditions for which providers might request a Geriatric Consult:

  • Falls and trauma root cause assessment and prevention.

  • Assessment of medications/reduction in polypharmacy.

  • Dementia assessment for ability to make decisions, discussing safety, as well as other behavioral issues.

  • Assessment of decision-making capacity which can include activation of POA or request for guardianship if needed.

  • Discussions about appropriate discharge with patients and family based on cognitive and functional status.

  • Assist with reducing repeated hospitalizations, at times offering a second opinion or support to the primary service in complex medical care.
